Your first visit to North Florida Center for Otolaryngology establishes a vital foundation for our relationship with you. During the first visit, we will obtain important background information about you and give you time to ask questions and get to know your doctor.

We know you have many choices when choosing an Otolaryngologist in St. Augustine, and we appreciate you entrusting us with your medical care.  Please contact our office at your convenience to schedule your appointment.  If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before your scheduled appointment time.

Being well-prepared for your appointment will ensure that the doctor has all of the necessary information to provide the best possible care for you. Please bring a list of your medications and any recent bloodwork or test results you may have.  Also, please bring a photo ID and your insurance card(s).  We look forward to your first visit.
